How to become a client in 6 steps
ONE STEP
- Are you or your loved one seeing
signs of dementia or Alzheimer's disease? - Visit your doctor – he or she will carry out tests
and make a diagnosis. - You can then consider the need for a residential service.
STEP TWO
- Submit your application at
www.alzheimerhome.cz/zadost - Fill in the application for admission
STEP THREE
- Contact your social worker.
- Visit a centre in person.
- Hand over the relevant medical reports and documents, and agree on an admission date.

STEP FOUR
- We will welcome you/your loved one at our centre.

- We will furnish and decorate a room together.
- This will be followed by an adaptation process
and gradual involvement in our programme and activities.

Visit a GP (neurologist, psychiatrist)
A doctor will carry out an examination and, based on the diagnosis, will prescribe medication that can alleviate or slow the disease progression.
Pre-Admission information
- Confirmed diagnosis of Alzheimer's disease or dementia
- This confirmation is provided by a GP, a neurologist or a psychiatrist.
- Care allowance
A social worker at our centre will help you fill in the care allowance application, or you can download it at MPSV (https://www.mpsv.cz/web/cz/-/zadost-o-prispevek-na-peci).
The allowance can be claimed by a person who, for reasons of long-term poor health, needs the assistance of another person in coping with their basic life needs.
The regional branch of the Labour Office decides on the granting of the allowance.
Für die Antragsstellung sind das Formular „Pflegegeldantrag“ und die „Mitteilung über die Pflegeperson“ (der/die pflegende Angehörige) auszufüllen. Ferner ist der Identitätsnachweis des Antragstellers vorzulegen.
There are 4 classes of care allowance:
a) class I (light dependence) – CZK 880/month
b) Pflegestufe II (mittelschwere Abhängigkeit) – 4.400 CZK/Monat
c) Pflegestufe III (schwere Abhängigkeit) –12.800 CZK/Monat
d) Pflegestufe IV (vollständige Abhängigkeit) –19.200 CZK/Monat
Der Leistungsanspruch entsteht durch die Eingabe des Pflegegeldantrags. Bei Aufnahme des Interessenten in die Einrichtung steht das zuerkannte Pflegegeld in voller Höhe der Einrichtung zu (gemäß § 73 Abs. 4. Ges. Nr. 108/2006 Slg., über soziale Dienste, in geltender Fassung).

In our facilities you will encounter staff in various coloured work attire. The colours represent the following professions:
Blue – healthcare worker
Pink - caregiver (social worker)
Green – activation worker
Light-grey – cleaning staff
Dark-grey - maintenance staff
